Adult Social Worker – Community Care Team
Liverpool, Merseyside | £32.82 per hour (Umbrella)
We are looking for an experienced Adult Social Worker to join a Community Care Team in Liverpool, supporting vulnerable adults to live safely, independently and with dignity.
Key Responsibilities

Complete Care Act 2014 assessments, reviews and support plans.

Undertake mental capacity assessments and contribute to best-interest decisions.

Respond to safeguarding concerns and undertake Section 42 enquiries.

Develop strengths-based, personalised care and support plans.

Assess and manage risks relating to abuse, neglect and vulnerability.

Work closely with NHS services, Occupational Therapists, mental health teams, housing and voluntary organisations.

Support adults, families and carers with information, advice and access to appropriate services.

Maintain accurate case records and prepare reports for panels and legal proceedings.

Promote independence, choice, wellbeing and positive outcomes.
Essential Requirements

Recognised Social Work qualification (BA/BSc/MA or equivalent).

Current Social Work England registration.

Strong knowledge of the Care Act 2014, Mental Capacity Act 2005 and safeguarding procedures.

Experience in adult social care, assessments, care planning and safeguarding.

Enhanced DBS clearance.

Full UK driving licence and access to a vehicle for community visits, where required.
