Eden Brown Synergy are currently looking for an experienced Qualified Social Worker to join the Hospital Pathways Team at Sutton Council.
The role involves supporting adults through hospital pathways and discharge planning, alongside safeguarding, Care Act assessments, mental capacity assessments, continuing healthcare assessments, and complex case management.
Duties and Responsibilities
Manage an allocated caseload within the Hospital Pathways Team, ensuring effective and timely case management.
- Triage allocated Adult Safeguarding cases and undertake safeguarding enquiry planning and coordination.
- Arrange and participate in multi-disciplinary meetings with health and social care professionals.

Complete comprehensive risk assessments across community settings, nursing homes, and care homes.
Undertake Mental Capacity Assessments and facilitate Best Interests Meetings relating to long-term care and support arrangements.
Complete Care Act assessments, identifying eligible needs and appropriate care and support arrangements.
Carry out Continuing Healthcare assessments jointly with professionals from the ICB.
Liaise effectively with residents, families, carers, health professionals, housing, and other partner agencies.
Support hospital pathway and discharge planning, ensuring appropriate care and support arrangements are in place.
Undertake Court of Protection applications for deputyship and Section 21A procedures for allocated cases where required.
Maintain accurate and timely case records, assessments, risk assessments, and reports in accordance with statutory requirements.
Participate in Duty responsibility approximately 2 days per month.
Working Pattern
- 3 days office-based at Civic Offices, Sutton.
- 2 days working from home.
- Hybrid working.
Pay Rate
£32 p/h Umbrella
Essential Requirements
- Social Work Qualification - Degree or equivalent.
- Social Work England Registration.
- Previous experience within Adult Social Care, particularly hospital pathways or hospital discharge services.
- Good knowledge of the Care Act 2014, Mental Capacity Act 2005, Best Interests decision-making, and Adult Safeguarding.
- Experience completing Care Act assessments, risk assessments, and safeguarding enquiries.
- Experience working within multi-disciplinary teams and alongside health professionals.
- Ability to manage a varied caseload and work effectively within a fast-paced environment.
- Strong communication, assessment, analytical, and report-writing skills.
