💙 About the Role

As a Care Coordinator, you’ll be responsible for coordinating care packages, managing staff rotas, supporting care workers, and ensuring service users receive the right care at the right time.

🔑 Key Responsibilities

Coordinate and manage daily care schedules and staff rotas.
Allocate care workers to service users, ensuring continuity of care wherever possible.
Respond to changes in service users’ needs and staffing requirements.
Maintain accurate care records, schedules and documentation.
Liaise with service users, families, care staff and healthcare professionals.
Monitor attendance, punctuality and service delivery.
Support recruitment, induction and ongoing communication with care staff.
Ensure care services meet safeguarding requirements and relevant quality standards.

Previous experience in care coordination or a similar role within the care sector.
Excellent organisational and time-management skills.
Strong communication and problem-solving abilities.
The ability to work well under pressure in a fast-paced environment.
A good understanding of care planning, safeguarding and person-centred care.
A professional, proactive and compassionate approach.
