We are looking for a dedicated and compassionate Band 6 Mental Health Practitioner to join our dynamic and supportive team. This is an exciting opportunity for an experienced mental health professional who is committed to delivering high-quality, person-centred care and making a meaningful difference to the lives of those we support. Working within a progressive service, you will play a key role in assessing, planning and coordinating care for individuals with complex mental health needs, working collaboratively with service users, carers and partner agencies to promote recovery, independence and positive outcomes.
Job Responsibilities:
· Working within a locality service and through participation in the local referral and care pathway, provide clinical assessment and appropriate triage with a view to risk assessment, risk management, safeguarding, sign posting, treatment or onward referral in response to identified patient need.
· Provide front line assessment to clarify the services the patients would most benefit from. This assessment may be undertaken jointly with other providers where it would be of benefit. This may be with the GP or with other statutory or non-statutory members of staff
· Where referrals indicate, work jointly with the local mental health services, specialist mental health teams and non-statutory teams to support prompt assessment and where clinically indicated, appropriate treatment plans.
· Design, develop and provide service information about the options available to patients.
· Following assessment, where indicate, provide treatment using effective evidence based intervention, as appropriate and participate in training and supervision to do so.
· Provide joint case reviews and care planning, should this be required, for patients with severe mental health problems that have now become stable.
What you’ll need:
· Appropriate degree level qualification, or equivalent, in Mental Health Nursing, Occupational Therapy or Social Work.
· Experience of care working in a mental health setting.
· Mental health assessment skills.
· Excellent communication skills, written and verbal.
· Experience of managing a case load.
