Hart Health Partnership is a large Practice in Fleet, Hampshire, serving a list of 28000 patients. We have a diverse multi-disciplinary team with 22 GPs, 11 Nurses, 4 paramedics, 1 ANP as well as other allied colleagues such as First Contact Physiotherapists, a Health & Wellbeing Coach, and Pharmacy colleagues.
We are a training practice hosting not only junior doctors and medical students but also nurse undergraduates and we would encourage the post holder to become involved in this as part of the ethos of the practice. We both have excellent workforces to which we wish to add a further Advanced Nurse Practitioner to support our Same-Day Urgent access team. You will be working alongside our Paramedic Practitioners in a hub model of collaborative working. Appointments will be a mix of phone calls and face to face, as well as taking professional calls from outside agencies.
Applicants must have completed a Centre for Advancing Practice (CfAP) accredited Master's (MSc) in Advanced Practice or the CfAP ePortfolio Supported Route. Clinical Competence Demonstrating advanced skills across the four pillars of advanced practice (clinical practice, leadership, education, research). Must be an independent prescriber. Typically requires 2-3 years of post-registration experience as a Registered Nurse (RN).
*ROLE AND RESPONSIBILITES*
The following are the core responsibilities of the Advanced Nurse Practitioner. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels.
* Consulting with patients either F2F, by phone or by econsult
* Supporting the duty doctor with medical complexity
* Supporting reception with care navigation
* Good communication with the team and patient
* Good use of IT systems- EMIS/Docmail/econsult/AccuRx
* Special interests welcome and will be nurtured eg joint injections, minor surgery, training & leadership
* To participate in extended access
* Support mentoring and supervision of students/colleagues/other health professionals as appropriate within skill set.
