Community Connector – Health and Wellbeing
The Role
Community Connector’s connect people with local community activities and services that can help improve their overall health and wellbeing, empowering people by focussing on what matters most, to them. You will be part of an integrated community team of trained staff triaging and signposting residents into community based support, or where appropriate statutory provision. You will take a holistic approach, focusing on the outcomes that an individual want to achieve, and helping the individual to achieve these outcomes through connecting them to appropriate community and statutory provision. This will involve the development of new partnerships whilst maintaining existing partnerships, planning and delivery of the activities to a high standard ensuring maximum impact in the community.
The connector role is central to developing a relationship with participants and can help to strengthen community and personal resilience, and reduce health inequalities by addressing the wider determinants of health, such as debt, poor housing, and physical inactivity; and by increasing people’s active involvement with their local communities. Specific to the role will be supporting participants aged 65 years and over to reduce social isolation, loneliness, building confidence and improving mental health with continuation of care and support to address the broader social factors of health and wellbeing.
You will be responsible for creating an action plan with participants to achieve their desired outcomes, and support them through delivery of the action plan, with staged assessments and redirection where appropriate.
You should be proactive and have the ability to be resourceful and creative to find the best support for participants and liaise with external organisations and build partner relationships. You will need a positive, ‘can do’ approach, working individually and as part of a team, communicating with people at all levels. You should have experience of working positively with people facing social and emotional challenges, and knowledge of solution-focussed coaching approaches. You will be working with people from diverse cultural and social backgrounds and will need to work confidently and effectively in a diverse and challenging environment.
We are looking for a friendly, engaging, empathetic individual with a natural ability to deliver with a compassionate and practical approach, putting the participant at the centre of the program.
You will have strong organisational skills, excellent verbal, and written communication skills. The ability to quickly build positive and supportive relationships. The ability to work with IT systems, input, and export accurate information and a professional approach to the presentation of information and communication. A strong commitment to team working with a proven ability to work flexibly as part of a team.
You will be required to travel within the North East Lincolnshire area at time, so you will be required to hold a full clean driving licence and have access to your own vehicle with insurance coverage for business use.

Accountable for: Supporting participants through the Community Support programme
Salary: £12,710.88
Hours: 20 hours a week to be agreed at appointment
Place of work: Centre4, 17 Wootton Road, Grimsby, DN33 1HE and other sites as needed, including home visits.
Annual leave: 21 days/year plus bank holidays pro rata, rising to 25 days/year – (pro rata)

Key Tasks and Responsibilities
- Deliver an effective Community Support Service to participants aged 65 years and over who are experiencing social isolation and loneliness across North East Lincolnshire.
- Plan and organise a programme of community activities, ensuring provision is successful and achieves targets
- Deliver community activity sessions to groups
- Meet with participants and carry out home and community visits as required.
- Undertake person-centred assessments to co-design health and well-being action plans with individual participants and identify support needs to ensure maximum engagement and improved health and well-being.
- Establish and maintain effective liaison with stakeholders including health, voluntary, social and education resources by attending relevant meetings and forums as required to promote the service.
- Work in partnership with voluntary and community sector organisations to understand local resources available to support effective community support with an aim at over 65 years and above who are experiencing social isolation and loneliness as well as other complex issues.
- Ensure information on sources of voluntary and community support is up to date to enable effective and accurate signposting by connecting service users with appropriate support and activities.
- Maintain accurate and comprehensive case notes and evaluation systems, including the Wellbeing Star and other IT recording systems as required.
- Adhere to confidentiality and information sharing protocols and design comprehensive outcome focussed reports detailing progress of participants.
- Work flexibly as required and engage with Centre4 meetings and activities to promote, support, and celebrate the work of Centre4.
- Evidence Centre4 values in your work at all times by being people focussed; reflecting a “can do” approach and striving to achieve excellence; also having mutual respect for everyone we work with, work for and support through our services.
- Ensure the Designated Safeguarding Officer is informed immediately of any concerns relating to safeguarding in relation to any incidents taken place on programme.
- Be a passionate advocate for all Centre4 projects
