Job Title: Intermediate Care Social Worker

Location: Hybrid working, meeting once a week in Winchester
Contract: Temporary - 3 Months
Our Intermediate Care Team forms part of the Younger Adults Service. We work countywide alongside the Community Mental Health and Substance Misuse Social Care Teams and the Approved Mental Health Professional (AMHP) service. Our team, including AMHPs and Best Interest Assessors (BIAs), provides a unique breadth of expertise, enabling us to support with diverse and complex needs, focusing on:
Enabling effective hospital discharge for individuals in Mental Health Hospital
settings, ensuring complex needs and risks are assessed and care planned
Transition from Children's Services/CAMHs to Adult Mental Health Social Care
Review of section 117 care needs in partnership with our health colleagues, with a focus on creative, strengths-based interventions which improve the outcomes for the people who use our service.
Your role
As an innovative and committed Social Worker, you'll be passionate about working with adults in a strengths-based way.
You'll collaborate with the people who use our service, their families and our partner agencies, to proactively encourage independent and safe living within the community.
You will be Supporting MH hospital discharges across Hampshire and Surrey Boarders
Need experience in MH and hospital discharge processes.
This is a hybrid working role, where you are expected to meet in Winchester once a week
This role is only available via Connect2hampshire as an agency
