Social Worker – Leaving Care Team
Wrexham | £38.43 per hour (Umbrella) | 6-Month Contract
Wrexham Children’s Services are looking for an experienced Social Worker to join their established and high-performing Leaving Care Team, supporting care-experienced young people aged 15–25 as they transition towards independence.
This is an excellent opportunity for a committed practitioner who is passionate about relationship-based, trauma-informed practice and helping young people achieve positive outcomes.
Key Responsibilities

Support care-experienced young people with transition and pathway planning.

Complete LAC reviews and statutory visits.

Promote independence, resilience and positive outcomes.

Apply corporate parenting principles and trauma-informed approaches.

Build strong, trusting relationships with young people.

Work collaboratively with families, professionals and partner agencies.

Model best practice and provide mentoring and support to colleagues.

Maintain accurate case records and ensure statutory requirements are met.

Apply relevant Welsh legislation, guidance and social care frameworks.
Essential Requirements

Recognised Social Work qualification – essential.

Experience working with children, young people or care-experienced young people.

Experience completing LAC reviews and statutory visits.

Good understanding of Leaving Care and transition services.

Knowledge of relevant Welsh legislation and children’s social care requirements.

Strong relationship-based and trauma-informed practice skills.

Ability to work effectively within a supportive, high-performing team.

Rate: £38.43 per hour (Umbrella)
Contract: Temporary – 6 months
