Licensed Vocational Nurse (LVN)
Provides direct and indirect patient care.
Provides direct and clinical procedures to the patient.
Performs assessments and evaluations of patient’s medical condition.
Teaches patients and their families.
Maintains communication with patient, family and physician.
Essential Functions:
The LVN complies with all organizational policies regarding ethical business practices and maintains regulatory requirements as governed by their licensing.
The LVN ensures compliance with policies and procedures regarding clinical operations, fire, safety, and infection control.
The LVN skillfully performs hands-on techniques and treatments necessary for safe patient care in accordance with professional standards.
The LVN completes regular skilled nursing visits performing appropriate preventive and rehabilitative nursing procedures.
The LVN inspects the assigned patient’s home environment for any possible dangers or hazards.
The LVN carries out patient’s current plan of care.
The LVN prepares professional clinical and progress notes and other documentation as required.
The LVN assists the registered nurse in performing specialized procedures.
The LVN responds in a safe and appropriate fashion to patient’s emergencies.
The LVN carries out physician orders.
The LVN administers medications accurately and documents it as per physician’s orders.
The LVN instructs patients and their family in learning appropriate self-care techniques.
The LVN acts as a resource for patients and their families.
The LVN informs the physician and other personnel of changes in the patient’s condition and needs, counsels the family and the patient in meeting nursing and related needs.
The LVN performs Aide visits as required.
The LVN participates in in-service programs.
The LVN attends case conferences and all other mandatory meetings.
The LVN rotates service on CQI Committee.
The LVN participates in continuing education programs.
The LVN conducts themselves in a professional manner, both verbally and nonverbal.
The LVN promotes a positive attitude at all times.
The LVN performs other duties as assigned or as needed.
Education:
Graduate of an accredited school of nursing
Fully licensed in the State of Texas
Training Requirements:
A minimum of two years nursing experience in home health services preferred.
Basic knowledge in computer use.
Must be CPR certified and have current TB skin test.
Preferred certification in performing home IV therapy procedures.
Training and experience in wound care, drawing blood, and technical catheter procedures.
Knowledge Requirements:
General nursing procedures and techniques involved in the care of patients, disease processes and possible results, medical terminology, HHA processes and equipment.
Must be able to read, write, speak and understand the English language.
Understanding of HHA basic functions and goals.
Basic understanding and knowledge of Medicare and Medicaid policy.
Basic understanding and knowledge of private insurances.
Able to make independent decisions when circumstances warrant such action.
Knowledge and mastery of Point Of Care device.
Professional Requirements:
Completes the annual educational requirements as required to maintain the LVN nursing license.
Maintains and ensures HIPAA compliance.
Responsible and accountable to the Supervising RN, the DON, the Assistant DON, the Administrator and the CEO regarding time availability and performance.
Maintains good rapport and cooperative working relationships with patients, physicians, referrals sources, regulatory agencies and staff.
Adheres to company dress code.
Attends annual review and departmental in-service training.
