JOB SUMMARY
Under the direction of the Clinical Coordinator or Manager and the Pharmacy Director, optimizes patient medications therapy by providing comprehensive clinical pharmacy services. Works collaboratively with the health care team to accomplish goals.
JOB REQUIREMENTS
Minimum Education
B.S. Pharmacy or Pharm D, or Master of Pharmacy
Continuing education as required for licensure by TSBP
Minimum Work Experience
Previous hospital experience with clinical responsibilities
Required Licenses/Certifications
Active registration as a licensed pharmacist in the state of Texas
Sterile Compounding Certification
ACLS or PALS
Registration as a preceptor with TSBP
Required Skills, Knowledge, and Abilities
Assess, document, and perform physician consultations, excluding total parenteral nutrition, but including all medication dosing consults.
Perform home medication reconciliation.
Provide education presentations to pharmacy staff and other staff upon request.
Ability to effectively direct and/or supervise personnel as appropriate.
Ability to handle confidential information.
Ability to maintain professional appearance and decorum at all times.
Ability to demonstrate excellent oral and written communication skills.
