RN Care Manager, flexible schedule – Trillium Woods
We are seeking a compassionate RN who has the very special gift of hospice nursing to assist our Residence Manager as the RN Nurse Coordinator at Trillium Woods, our beautiful 20-bed inpatient hospice residence.
What will you do?
Work a schedule that complements the Residence Manager
Responsible to assist in the supervision of the daily operations at our 20-bed hospice residence, being present on the floor and assisting the care team when needed
Participate in the Hospice interdisciplinary group meetings
Case manage doing weekly assessments, recertifications, and care conferences
Assist with admissions when needed
Backfill when the Residence Manager is out of the building
Assist Residence Manager with hiring, orienting, and ongoing coaching of the care team
Assist with covering for open shifts when needed
Participate in the manager on-call rotation with the other hospice managers
What is needed to be successful in this role?
This opportunity requires licensure as an RN in the State of Michigan
Minimum of 1 year of nursing supervision in a skilled nursing or med/surg setting
Prior management and hospice nursing experience is highly preferred
Strong computer skills to chart in the EMR are essential.
