Shift: Monday - Friday 5:00am - 1:00pm and Saturdays as needed 6:00am - 10:00am
Community Medical Services (CMS) is hiring an Opioid Health Home Nurse Care Manager. This position is responsible for promoting and developing a person-centered health care plan through a collaborative process that assesses, coordinates, monitors, and evaluates services that address the individuals' health care needs while promoting quality and cost effective outcomes.

Responsibilities:
We're looking for someone who:
Has experience working with multi-cultural and low socio-economic individuals.
Is excited to work in an outpatient setting, much like a doctor’s office.
Brings residential experience and knowledge of psychotropic medications.
Responsibilities:
Provides ongoing information and support to caseload of clients and their families and caregivers.
Maintains strong relationships with system partners and providers throughout the health and human service community.
Conducts comprehensive assessment independently or as member of an interdisciplinary team.
Develops, implements, and monitors person-centered care plans in accordance with established guidelines and standards.
Evaluates the effectiveness and benefits of the services provided to participants.
Provides motivational interviewing and coaching to help participants and caregivers achieve optimal social and health outcomes and goals important to them for quality of life.
Monitors behavioral patterns, medical symptoms, and physical symptoms related to dosage, as well as other
symptoms related to drug use (both licit and illicit) and mental health concerns
May obtain vital signs, blood specimens, and urine samples
May process point of care urine drug screens per company policy
Provides advocacy and facilitates formal and informal care and services through external sources.
Assists with administer dosing to clients following orders by a medical provider
May assist with ordering nursing or other supplies necessary to carry out clinic functions
Follows provider orders, as applicable
May order specialty medications, as required, for treatment of STIs or other conditions
Qualifications:
Licensed RN in state(s) of employment
CPR Certification Required
First Aid Certification Required
Ability to travel to assigned CMS sites
Experience in the field of substance use disorders preferred, but not required
Basic to intermediate computer knowledge
Knowledge of the signs of intoxication and opioid withdrawal symptoms
Demonstrates accuracy in medication administration and ability to maintain accurate client records
Problem solving, conflict resolution, and strong customer service skills

Excellent organizational skills, accuracy, and attention to detail
Culturally competent and sensitive to client and employee needs
Ability to communicate clearly and effectively between all organizational levels and with outside providers

Tools and Equipment Requirements
The ability to use a phone, computer, printer, and copier is required.
Frequent use of Microsoft office products, including but not limited to Outlook, Word, Excel, and PowerPoint.
The ability to use the internet and various web browser software is required.
Physical Working Conditions and Office Setting Description
Requires sitting and standing associated with a normal office environment.
Manual dexterity using a calculator and computer keyboard.
Requires prolonged sitting, standing, frequent bending, stooping, or stretching.
Frequent and prolonged typing and frequent and prolonged operation of computer, keyboard, and telephones required.
Requires occasional use of fax machines, telephones, copiers, and other office equipment.
Employee must regularly lift and/or move up to 10 pounds, frequently lift and/or move up to 25 pounds and occasionally lift
