Transitions Social Worker - Staffordshire
Base: Stafford | Caseload: Staffordshire (kept as close to home as possible)
Are you a qualified Adult Social Worker who thrives on meaningful work and real impact? Join a dynamic Transitions Team supporting young people with disabilities, neurodiversity and mental health needs as they move into adulthood.
This is a fast‑paced, rewarding role where your expertise will directly shape outcomes for young people and their families across Staffordshire.
What You'll Be Doing
Managing a varied Transitions caseload, including reviews, safeguarding enquiries (S42) and late referrals
Completing strengths‑based Adult Social Care assessments under the Care Act 2014
Working within the Mental Capacity Act, Mental Health Act, and DoLS
Supporting young people with learning disabilities, autism and co‑morbid mental health needs
Developing outcome‑focused support plans that promote independence and choice
Identifying and managing risk confidently and proportionately
Working closely with families, carers, providers and multi‑disciplinary partners
Contributing to MAPPA, MARAC and S117 work where required
Keeping clear, high‑quality case records and contributing to team performance targets
What We're Looking For
Qualified Social Worker with Social Work England registration
Experience in Adult Social Care (Transitions experience ideal but not essential)
Strong knowledge of legislation and confident safeguarding practice
A motivated, person‑centred professional who delivers quality work
