We are recruiting a skilled and compassionate Social Worker to join the Hospital Social Work Team in West Lothian. This is a rewarding opportunity to work within a supportive, experienced, and multidisciplinary hospital-based team, helping adults and their families during times of significant change.
The Role
As a Hospital Social Worker, you will:

Complete strengths-based assessments to support safe and timely hospital discharge

Work closely with health colleagues across wards and community services

Support adults and their carers to make informed decisions about care and support

Undertake capacity, risk, and safeguarding assessments where required

Contribute to adult protection work and complex decision-making

Maintain accurate, high-quality case records and reports
-
Promote independence, dignity, and positive outcomes for service users
What We’re Looking For

Degree or equivalent qualification in Social Work

SSSC registration (or eligibility)
-
Experience in adult social work (hospital or health setting desirable)

Strong assessment, communication, and multi-disciplinary working skills

A calm, professional approach in fast-paced environments
