Care Assistant- Adult Community Support Services – Oxfordshire

Pay Rates: £12.21 - £13.90 per hour umbrella rate
Information the Role: This is a rewarding opportunity to support people with learning disabilities, Autism, physical disabilities, ageing health needs and mental health needs to live fulfilling lives within their communities. This requires flexibility to work across service locations in Oxfordshire. As a Care Assistant, you will deliver high-quality, person-centered support to individuals, ensuring statutory requirements are met and that people are fully involved in their own planning and decision-making.
What you will be doing:
Person-Centered Support & Daily Care
Ensure individuals are fully involved in their person-centered planning, with plans developed and shared with their support networks.
Provide personal care and healthcare support as required.
Support individuals diagnosed with Autism by developing environments and approaches tailored to their needs.
Empower people to have choice and control within a regularly reviewed risk management framework.
Support individuals and groups to access and prepare for activities including sport, leisure, recreation and college courses.
Use Intensive Interaction, Makaton and Total Communication methods as required.
Assist with daily living tasks including personal care and meal preparation.
Motivate and encourage people to achieve their identified outcomes.
Health, Safety & Administration
Take responsibility for health and safety, vehicle checks and risk assessments.
Administer, store and accurately record medication following appropriate training and policy.
Support and record finances where required.
Maintain accurate, up-to-date records including Whole Support files and activity records.
Report and record accidents, incidents and near misses appropriately.
Undertake manual handling duties safely.
Safeguarding
Safeguard and promote the wellbeing of vulnerable adults in line with policies and procedures.
Identify and raise Safeguarding Alerts where necessary, ensuring timely escalation.
Maintain confidentiality and adhere to information-sharing protocols.
Community Integration & Partnership Working
Maximise the use of community resources and voluntary sector partnerships.
Work collaboratively with internal and external services across Oxfordshire.
Promote strong multi-agency relationships to improve outcomes.
Support individuals to use the Council’s Comments and Complaints policy when required.
Professional Development
Take ownership of your professional development through training, supervision and appraisal.
Share knowledge and expertise within the team.
Act as a champion in an identified area of special interest.
Participate actively in team meetings and service development initiatives.
Service & Performance
Contribute to service development through ideas, briefings and team discussions.
Support team performance targets.
Work flexibly across Community Support Service locations as business needs require.
Essential Skills & Experience
Extensive experience supporting people with learning disabilities, Autism, physical disabilities or ageing health needs (paid or voluntary).
Proven ability to deliver person-centred planning and individualised support.
Experience working with individuals diagnosed with Autism.
Good literacy, numeracy and IT skills.
Accurate record-keeping ability.
Knowledge of safeguarding and confidentiality protocols.
Strong communication and interpersonal skills.
Ability to build trust through empathy and active listening.
Ability to work independently and as part of a team.
Calmness under pressure and effective workload management.

Manual handling capability.
Qualifications
NVQ/QCF Level 2 in Health & Social Care, Care Certificate or equivalent.
Enhanced DBS (required).
