Role Overview
We are currently seeking an experienced Band 6 Physiotherapist to support the Urgent Community Response (UCR) and Admission Avoidance & Discharge Service within East London.
This role is based at the Royal London Hospital and across the community in Tower Hamlets, working within a multidisciplinary team to support early discharge, rapid response, and prevention of unnecessary hospital admissions using a Discharge to Assess model.
The postholder will rotate across:
Rapid Response (including Falls Pick-Up Service)
Intermediate Care
Admission Avoidance within Emergency Department
Key Responsibilities
Deliver specialist physiotherapy assessments and interventions across acute and community settings
Support admission avoidance and safe, timely discharge from ED and short stay units
Manage a caseload of complex patients with physical, cognitive and psychosocial needs
Work autonomously while contributing to MDT decision-making
Arrange and install equipment at short notice to facilitate safe discharge
Provide rapid community follow-up post-ED discharge
Contribute to service development, audit and quality improvement initiatives
Essential Requirements
Degree or Diploma in Physiotherapy
HCPC registration
Minimum 18 months post-qualification experience
Experience in acute, community or intermediate care settings
Strong clinical reasoning and autonomous practice skills
Ability to work flexibly, including late shifts and weekends if required
Desirable
Experience within UCR / ICT / Admission Avoidance services
Experience with equipment provision and discharge planning
CSP membership
