Family First Case Manager
Knowsley
Umbrella Pay - £21.48 per hour
5 months initially
Monday to Friday, 9am – 5.30pm, 36 hrs/week
Responsibilities
- Direct work with children, young people and families
- Act as the lead professional and main contact for the family
- Deliver targeted and co-ordinated interventions to families with children 0-18 years displaying a range of complex needs
- Support to achieve better outcomes including improved school attendance and tackle issues such as truancy and exclusion
- Complete Early Help Assessments and SMART plans with families and then review
- Complete home visits
- Engage with families using all options available to ensure that their needs are met
- Organise Team Around the Family Meetings
- Keep and maintain accurate records of work
Requirements
- Experience of case management including undertaking whole family assessments to identify risk and need
- Knowledge and understanding of key policy drivers and recent reports addressing early intervention and prevention with young people and families with multiple needs
- Experience of working with children and families to deliver preventative interventions
- Experience completing Early Help Assessments and SMART plans
- Child DBS registered to the update service
