Personal Advisor - After Care Team
Oldham

Monday to Friday, 08:40 - 17:00, 36 hrs/ week – Hybrid
Our client is seeking a Personal Advisor to join an After Care Team based in Oldham.
- Direct work with young people aged 16 – 25 years who are or have been looked after or accommodated by the local authority
- Assist the young people to live an independent life in their community
- Maintain regular contact with young people
- Implement and review individual pathway plans for young people to meet assessed needs and risks
- Develop a local knowledge and understanding of resources available to meet the needs of the young people
- Engage with internal and external organisations as required to encourage and secure permanence in accommodation, education, employment, training, and health
- Act as an advocate for the young people
- Ensure that their views and wishes are taken into account when developing Pathway Plans, promoting their involvement at all times
- Develop strong, trusting working relationships
- Ensure files and records are accurately maintained
- Engage in regular team meetings and participate positively in supervision with the Senior Practitioner or Service Manager
Requirements:
- UK Driving Licence, access to a vehicle, business insurance
- NVQ Level 3 in a related subject such as Child Care or Health & Social Care or equivalent
- Recent experience of working with young people leaving care
- Evidence of commitment and determination to help young people achieve their aspirations
- Experience of keeping accurate records, writing reports while updating and reviewing care plans
- Excellent written and verbal communication skills
- Ability to develop working relationships
- Enhanced Child and Adult DBS registered to the update service
