The Specialist Dementia & Mental Health Lead will provide strategic clinical and operational leadership to Age UK NT specialist dementia services and those associated with supporting people with other cognitive impairments in later life.
They will work with key stakeholders to develop and deliver the Age UK North Tyneside’s Mental Health & Wellbeing in Later Life Strategy as well as having potential to work in partnership with Dementia UK.
Main Duties and Responsibilities include:
Expert Clinical Practice
Provide expert clinical practice based on evidence-based approaches to care, to meet the needs of people with dementia , their carers/families and staff delivering the care.
Support the delivery of specialist nursing assessment, using the Admiral Nurse Assessment Framework, for carers/families.
Ensure the delivery of a range of bio-psychosocial interventions to promote health and wellbeing for people with dementia and their carers/families across all of AUKNT services.
Ensure that the needs of service users/ carers/ families are met from first contact with the service to post bereavement support, paying particular attention to loss, transitions and changing relationships.
Support the improvement of identification and management of co morbidities and frailty for people with dementia and their carers/families.
To maintain contemporary clinical e records in line with local policies and NMC Code of Professional Practice.
Professional Leadership: specialist advice and guidance
Work in collaboration with key stakeholders to develop, deliver and evaluate the impact of the Age UK North Tyneside’s Mental Health & Wellbeing in Later Life Strategy.
Lead Age UK North Tyneside’s specialist dementia and mental health service delivery working in partnership with other officers to do so.
To take an active role in the delivery of the organisation’s mission, vision, and values, providing inspirational leadership, direction, and purpose
Represent the organisation to strategically influence policy and practice for the benefit of the people we serve.
Represent Age UK North Tyneside and Dementia UK on national, regional and local groups to advance practice, improve outcomes and enrich the care experience of families/carers with dementia.
Education, Training and Research
To ensure the delivery of evidence-based education and training programmes to support Age UK North Tyneside staff to develop, flourish and provide high quality care.
Work collaboratively with the Admiral nurse team and key stakeholders to ensure that Age UK North Tyneside is a recognised as a positive learning environment for student nurse placements.
Contribute as appropriate to pre and post registration higher education programmes at local universities.
