*Essential Requirements: *
* In depth knowledge of person centred, asset based support planning and risk assessments including knowledge of safeguarding children and adults.
* Awareness of factors that put people at risk of homelessness
* Awareness of factors that put people at risk of hospital admission and / or delayed discharge
* Skilled at IT and updating case management systems
* Experience of working with a diverse range of people and working effectively in partnership with others
* Level 3 NVQ qualification or equivalent work level experience.
*The Role: *
As a Referral and Allocations Partner will receive direct referrals and enquiries, providing a single point of contact for the Social Reablement and Crisis Support Service, working alongside partner agencies to ensure people accessing the service receive the correct support in a timely manner
You will work in conjunction with the Home from Hospital Service ensuring the service is as easy to access as possible.

The appointed Referral and Allocations Partner will:
* Be person centred at all times and work with people to identify what is important to them and ensure the most appropriate intervention provides an effective route to support.
* Deliver a ‘no wrong door’ approach, providing a supported referral into other services where more appropriate
* Work directly with the Social Reablement team, allocating cases effectively to the most appropriate intervention
* Work collaboratively with statutory and non-statutory services including but not limited to Hospitals, CMHTs, Substance Misuse Teams, Healthcare professionals, social prescribing initiatives, early help and preventative approaches of partner agencies, providing a coordinated and holistic service involving advocacy and crisis response.
