Responsibilities
The Information Coordinator provides data entry and coordination efforts related to required data management for the Health Homes Program. Also provides registration, reception and billing services for the HOPE Center in accordance with the Department’s and hospital’s mission and philosophy.
Provide and maintain data entry services for the HOPE Center, which include data entry into AIDS Institute Reporting Services (AIRS), E-share, MS-Access GSI database and others, as needed. Access pertinent information from the Meditech system, The Health Commerce System, ePACES and others as needed. Compile and print database searches and reports on a regular basis, including special request reports from the AIRS/Meditech,/Access. Assist in preparation of monthly statistical reports required by the AIDS Institute. Serve as a liaison between HOPE Center Primary Care, Psychiatric, Dental services and program management to assure that the data coordination function operates smoothly and effectively to support the program’s overall goals. Provide Registration coverage for HOPE Center-Primary Care during . hours/lunch daily and in absence of HOPE Center-Primary Care Registrar. Provide Registration coverage for Dental Clinic. Schedule patient primary care appointments, in-house specialty appointments and perform functions to remind patients of appointments. Relay any changes to appropriate personnel. Verify client’s Medicaid numbers on ePACES report all insurance status changes to appropriate staff members. Prepare and complete planned visits for following day and billing in absence of Registrar at HOPE Center. Responsible for the confidentiality of all materials handled in the performance of above duties. Assumes other responsibilities when assigned.
Responsible to actively participate in the Department’s Quality Improvement and Quality Assurance efforts as directed by their supervisor, including but not limited to participation on the Department’s Quality Improvement Committee. Competent to work with clients appropriate to the setting of the program and with special populations. Responsible for maintaining data for the HOPE Center’s Health Homes program, which include data entry into GSI and retrieving and formulating data from other HH platforms, as needed. Access pertinent information from the Meditech system as needed. Responsible for maintaining an accurate program roster across all platforms used by the Lead HH programs: BTQ/GSI /Health Commerce System; Maintain accurate caseload rosters in Excel for all HH Care Managers and manage program’s referral tracking log. Work as a team member within the HH program to support the day to day workflow and management of client information and records. Monitor Health Homes member’s HOPE Center primary care / psychiatric / dental / specialty appointments, and inform appropriate HH Care Management staff. Assist in acquiring required documentation for program eligibility. Create new member files (electronic and hard copy). And other duties as needed. Verify client’s Medicaid status via ePACES and BTQ at the beginning of each month for the entire HH roster. Report all insurance status changes to appropriate staff members. Responsible to manage the workflows of: member referrals, intakes and closings, following each of the Lead HH policies and procedures as well HOPE Center’s. Work with program administration to ensure billing reconciliation is accurately completed on a monthly basis.
Qualifications
High School Diploma or Equivalency. Additional certificate(s) in training in business or public health/administration. Experience with Meditech (hospital’s EMR). Bilingual (Spanish/English), Preferred. Working knowledge of computer software including database management, MS Suite: Word/Excel/PowerPoint Required. Possesses well-developed and effective interpersonal skills and is able to communication in writing. Demonstrates confidence in actions and exercises good judgment. Embraces and adapts to change. Familiarity with medical terminology. Ability to work independently yet collaboratively within a larger system. Exercises good judgment.
