The role is offered at 32 hours per week although flexibility on hours will be considered. You will be based from home with required, regular working at our head office location in Amesbury, Wiltshire (Hybrid).
About the role:You will manage the referrals process for all case management clients, ensuring professional and efficient case allocation. You will manage quotations for prospective clients, coordinate and quality check Immediate Needs Assessments (INA), and oversee client introductions (Meet and Greets). Collaboration with the Head of Case Management and the Head of Clinical Compliance will be key in delivering the highest quality services to our clients.
Additionally, youll be responsible for managing learning, development, and Continuing Professional Development (CPD) activities for case managers. You will lead projects and implement events and strategies that support the companys objectives.
What we are looking for in you:- Experienced healthcare professional with current registration (HCPC, NMC, or Social Care England).
- Significant experience in case management, particularly in the referral process.
- Proven clinical leadership and project management skills.
- Ability to coordinate learning and development events.
- Excellent communication and interpersonal skills to maintain professional relationships with clients, referrers, and colleagues.
- A positive, supportive approach to delivering high-quality, client-centred services
