Optimal Health Care Inc. provides Health Care Case Management in patients’ homes throughout Cecil County. We are looking for a motivated Physical Therapist to join us, who has an interest in healthcare case management and a desire to reduce readmissions for patients with chronic illnesses.
The Physical Therapist (PT) will work with our skilled Clinical Care Director and RN to provide our clients the professional support they need to continue recovering in their own homes, and maintain their health.
If you are a mature, independent physical therapist (PT), highly organized and efficient with excellent skills, interested in serving your patients in their homes…please apply!
