*Home Health and Hospice RN Case Manager needed in Virginia!*
*Pay Rate:* $34.00/hour
*Shift: *Days, 5x8's, flexible schedule
*Location:* HYBRID (remote and out in the field). Paige, Shenandoah, Augusta, and Rockingham Counties service area
*Contract:* 13 weeks or Permanent Placement
*Requirements: *BLS, 1 year of RN experience (Home Health/Hospice preferred)
*JOB DETAILS:*
* Work independently with groups of clients to provide initial and on-going assessment of the client’s skilled needs.
* Implement individual plans of care and make referrals to others disciplines and /or revise the plan as necessary.
* Provide ongoing consultation and education for patient/family members regarding all aspects of care.
* Communicate and collaborate with family/caregivers and other professionals involved in the care of the client.
* Supervise Licensed Practical Nurses and Home Health Aides.
