Prism Health and CAP are excited to announce that we are hiring an experienced Registered Nurse I. The Registered Nurse (RN) I is an integral part of a multidisciplinary team that provides comprehensive healthcare to patients at Prism Health. The RN works autonomously and in collaboration with the clinical and integrated behavioral health team to provide comprehensive patient care, health education, and disease prevention using a trauma-informed approach and evidence-based approach. The RN also plays a key role in the Quality Improvement metrics throughout the clinic and collaborates with other members of the Quality Improvement Committee to improve and implement clinic workflows and ensure compliance with Federally Qualified Health Center (FQHC) policies, procedures, and quality assurance standards.
This is a full-time role working in our Portland clinic on SE Belmont Ave or NE Morris St, adjacent to Legacy Emmanuel (opening in June 2024). The Starting pay is $89,523.20 a year. The Registered Nurse's compensation increases with tenure with the top of the range being $97,323.20. Clinic and work hours are Monday to Friday, 8 am to 5 pm. A full-time schedule includes 32 hours of direct patient care and 8 hours of admin time each week. Travel throughout the county and evening and weekend work are occasionally required. The compensation, benefits, and conditions of work for this position are collectively bargained.
Who you are:
Minimum 1 year of nursing experience
Bachelor of Science in Nursing (BSN)
Basic Life Support certification
Valid Oregon and Washington RN license
3+ years nursing experience
Experience working within an FQHC or a community health setting.
What you'll do:
Provide comprehensive nursing care to all ages in the FQHC setting.
Work in collaboration with other members of the healthcare team to provide trauma-informed, patient-centered, and evidence-based care.
Conduct comprehensive assessments of patients including physical exams, vital signs, phone triage, medication reconciliation, and social needs assessments.
Administer medications, vaccines, and other treatments as prescribed by providers or in accordance with existing standing order protocols.
Perform nursing visits based on nurse visit protocols.
Perform triage, both in person and over the phone.
Other duties deemed necessary to achieve the mission of the organization.
