The DHS Nurse is responsible for providing leadership in all aspects of the provision of nursing services while ensuring regulatory compliance and service excellence. While this position is ultimately responsible for ensuring that an RN is available to staff 24/7, Ebenezer provides on-call/afterhours nursing support to our assisted living sites using an efficient and effective triage system. A trained triage nurse is available by phone to provide evaluation and direction for unexpected medical situations occurring at the community during evening and overnight hours. This leadership nursing position supports the entire Assisted Living staff within this Community and serves in a manager on duty role as needed. Supported by caregivers- Resident Assistants whom are trained to pass medications.

Responsibilities/Job Description:
Oversees Resident Centered Care:
Ensures the development of RN assessments and the implantation of individual resident care plan/service agreements that attain or maintain the highest level of physical, mental, and psychosocial well-being of the resident. Ensures that care planning incorporates Honoring Choices (POLST) and Resident Bill of Rights.
Ensures appropriate resident/family participation and education in the care planning/service agreement processes. Sees that this is properly documented.
Ensures Clinical Compliance
Effectively plans, organizes, directs, and implements comprehensive health care program. This includes, but is not limited to, staff placement consistent with scope of practice, medication administration program, resident safety, and staff development.
Collaborates with Ebenezer administration to develop nursing policies and procedures that reflect current clinical practice, professional standards, and Ebenezer’s philosophy of care.
Evaluates and modifies goals, objectives, and organizational structure of the health services department to continually provide health care services consistent with best practices.
Oversees site compliance with all applicable regulations including, but not limited to, WI Board of Nursing, Wisconsin State Home Care Licensure, HIPAA, OSHA, CLIA, & Medicaid.
Advises administration in matters related to nursing services and environmental safety enlisting compliance specialists as appropriate.
Develops and participates in the implementation of a process that collects and analyzes health services data, identifies trends, institutes actions to resolve problems promptly, and evaluates effectiveness of these actions.
Manages nursing department budget:
Participates in budget development and monitors budget compliance.
Prepares financial/statistic reports as requested.
Provides leadership and general oversight of the nursing department:
Develops goals with nursing staff and holds them accountable to their job responsibilities.
Provides on-going support to and encourages professional development of leadership team.
Establishes and monitors an effective staffing model and ensures efficient use of staffing resources.
Oversees delivery of all services provided by care providers and ensures staff compliance with all pertinent regulations and Ebenezer policies and procedures.
Establishes on-going training programs for nursing staff consistent with applicable local, state, and federal regulations.
Monitors employee satisfaction and leads the development of appropriate employee engagement strategies.
Ensures 24-hour availability of RN on-call.
Performs essential functions of any health care team member if required.
Leads and/or Performs Coordination of Resident Care:
Ensures availability of adequate tools to support implementation of resident care plan. This includes supplies such as medications and durable medical equipment.
Encourages collaboration with community resources to meet full complement of resident identified needs.
Ensures accurate and timely communication of resident care plan needs with all internal departments such as dietary, billing, etc.
Ensures collaboration among resident, family, physicians, consultants, community agencies, and institutions to facilitate safe transfer of care as needed.
Ensures that assessments are consistently completed in a timely manner and that MD orders are updated to validate care plans.
Supports a marketing plan aimed at full occupancy:
Strives to create a culture of services excellence through role-modeling, training and coaching nursing staff in the principles and practices of customer service.
Collaborates with Ebenezer marketing team and assists with marketing and outreach initiatives. This may involve visits to referrals sources, public speaking, and individual resident/family consultations.
Collaborates with the management team to develop and maintain a system for soliciting customer feedback and following up on concerns and suggestions.
Qualifications:
Required:
RN Licensure/Registered with the MN Board of Nursing
Vocational/Technical Training or Associate Degree
Current Basic Life Support (BLS) for Health Care Provider from American Heart Association (AHA) or American Red Cross;
accepted certifications as follows -AHA: BLS for Healthcare Providers (CPR & AED) Program or BLS Provider - BLS Instructor.
American Red Cross: CPR/AED for Professional Rescuers and/or Healthcare Providers, Life Guarding First Aid/CPR/AED
One to three years experience
Preferred:
