Job Overview::The Assessment and Referral Clinician specializes in clinical screening and evaluation of behavioral, emotional, mental, and substance use disorders and facilitates seamless referrals to internal behavioral health services throughout the consortium. The clinician is highly organized and possesses strong writing and documentation skills. The clinician is experienced and comfortable working with a diverse range of mental and behavioral presenting problems serving all ages and has the clinical ability to engage with patients and families efficiently and effortlessly.
Responsibilities::
* Provides behavioral health screenings and assessments with patients, family members, and other key treatment team members (case manager, OCS, DJJ, etc.).
* Performs safety assessments, and when indicated completes a safety plan in accordance with SEARHC standards, this may require crisis intervention/stabilization services.
* Provides treatment planning in coordination with the patient, family, and other key team members.
* Documents assessments and treatment plans timely, clearly, and concisely in the electronic health record per SEARHC standards that comply with all accrediting regulatory agencies (The Joint Commission, Division of Behavioral Health, Centers for Medicare and Medicaid, etc.)
* Maintains all records and patient information in a confidential manner according to SEARHC standards.
* Develops a strong working knowledge of SEARHC communities and internal and external behavioral health clinical resources to facilitate care and referrals efficiently and effectively.
* Fulfills minimum productivity standards as set by the department clinical manager.
* Collaborates and coordinates with internal SEARHC departments and outside community social service resources to assure appropriate services are utilized for optimum patient care.
* Actively participates in staff trainings/meetings.
* Licensed professionals are responsible for clearly understanding and following licensure (or certification) expectations and standards for initial licensure and renewal requirements.
Qualifications::*Education,* *Certifications, and Licenses Required*
* Master’s Degree in Social Work, Psychology, Counseling, or related discipline from an accredited educational institution.
* Licensed as Clinical Social Worker (LCSW), Professional Counselor (LPC) or Marriage and Family Therapist (LMFT) preferred.
* Once licensure is obtained it is a requirement to keep it current for the duration of employment.
* Clinicians who are not yet licensed will receive clinical supervision for behavioral health services focused on providing the appropriate delivery of core functions: Screening, intake, orientation, assessment, treatment planning, therapy, case management, crisis intervention, patient education, referral, reports, and records keeping, and consultation.
* Valid State of Alaska Driver’s License or can be obtained within 6 months of hire.
* Alaska State Chemical Dependency Counselor I or MAC certificate or equivalent preferred.
*Experience Required*
* Experience and expertise with behavioral health assessment and diagnoses.
* Ability to provide assessment, treatment plans, and referral for patients of all ages.
*Knowledge, Skills, and Abilities:Knowledge of*
* Behavioral health assessment and diagnostic criteria.
* Initial treatment plan development and patient centered goals and objectives.
* Evidence based modern methods, techniques and procedures used in individual, group, marriage, family and child counseling for mental health and substance-using patients.
* Federal and state mandatory reporting requirements
* Ethical and legal management practices, patient rights, documentation regulations specific to The Joint Commission, HRSA, State Medicaid and Federal regulations
*Skill and Ability to*
* Assess patients for risk of suicide or self-harm and develop effective safety plans.
* Advocate for patients and coordinate required services or to resolve problems in crisis situations.
* Analyze, evaluate, and diagnose situations and adopt effective courses of action.
* Document clear/concise reports drawing from direct patient observation, screenings, and assessments in accordance with professional standards, state regulations and department protocol.
* Modify treatment activities or approaches as needed to comply with changes in patients' needs.
* Maintain knowledge of new developments in behavioral health assessment and counseling.
