*Physical Therapist*

*Responsibilities:*
* Develop and implement physical therapy plans of care ensuring quality and safe delivery of physical therapy home health care services.
* Work directly with patients to achieve maximum physical recovery.
* Evaluate effects of therapy treatment and communicate patients' progress.
* Record and document patient care services.
* Promote education for clients, client families and the community.
* Coordinate provision of physical therapy home health care services that reflect VNCC’s philosophy and standards of home health for assigned clients
*Qualifications:*
* Current PT licensure in MA required.
* Two (2) years experience in home health care, elder services, and/or public health preferred.
* Strong assessment skills needed.
* Excellent communication skills with a customer service orientation necessary.
* Ability to build rapport with patients.
* Ability to handle physical workload.
* Work with a friendly and supportive team of seasoned clinicians at an agency that understands the need for work/life balance.
