Who You Are Experienced social worker required to join the Integrated Discharge Team (Contact Cares) based in Whiston Hospital. You should have experience in fast-paced, high-pressure environments, preferably within hospital discharge processes. What the job involves This full-time role requires assessing patients and executing tasks to facilitate their discharge from the hospital. The aim is to ensure patients receive the support they need to either return home or be discharged to an appropriate setting. The position involves working 37 hours per week, between 8.45am and 5.15pm, with a combination of in-office and remote work, including ward visits to assess patients as necessary.
