Clinical community liaison and co-ordinator It is essential that the Clinical community liaison and co-ordinator role is filled by an individual with the cultural, language and background of South Indian communities, especially of Tamil and Malayali descent. Sante Care primarily serves the population of East London which has a significant number of deprived and isolated ethnic groups. The main ethnic groups who pass through our service and who ‘are hard to reach’ are people from the Tamil and Malayali community. Job Description: To act as a liaison between healthcare providers, community organizations, government agencies and the patient to ensure that the needs of patients are being met. To educate and empower individuals from the Tamil and Malayali populations to take control of their own health and advocate for policies and programs that promote health equity. Work in Sante Care clinics, liaise with the patient and the patient’s family, offer translation services, encourage the patient to engage with the health professionals. To signpost and explain secondary care organisations and work directly with specific Tamil and Malayali populations. The goal is to promote health and well-being in the Tamil and Malayali community by addressing the social determinants of health such as access to healthcare. Through their work, the individual can help to reduce health disparities and improve the overall health of their communities. Essential criteria Exposure to clinical health care Experience in working in hospitals and clinic settings Experience in engaging patients with care in the community Knowledge of Tamil and Malayalam languages as a native speaker Knowledge of the Tamil and Malayali culture and community norms as a native Desirable criteria Engaging with local health initiatives, volunteering, and gaining certifications in community health work. Effective communication, empathy, and a deep understanding of community needs are crucial as advocates work to bridge gaps in health care, promote wellness, and empower individuals to lead healthier lives.
