Job Title: Qualified social worker

Rate: £32 per hour
The London Borough Of Lambeth Council is currently recruiting for an experienced and qualified Social Worker to join their Intermediate Care Team which is an Integrated service with Adults Social Care and NHS services for adults and Older people, offering a pay rate of £32 per hour.
What will your responsibilities be?
In this role, you will prevent service users from being admitted to the hospital and provide rehab and reablement to service users in the community. You must be exceptional in delivering services to service users. Additionally, you will complete assessment home visits, safeguarding Section 42 enquiries, carers assessments, mental capacity assessments, attend and participate in multi-disciplinary meetings (MDMs), cover duty, and complete urgent visits.
