*Key Responsibilities:*
* Provide high-quality patient care, including chronic disease management, immunisations, wound care, cervical screening, and health promotion.
* Assist in developing and implementing practice protocols to ensure compliance with clinical guidelines.
* Conduct health assessments and screenings to identify patient needs and provide tailored care plans.
* Collaborate closely with GPs, healthcare assistants, and other members of the clinical team to ensure integrated and holistic care.
* Maintain accurate and up-to-date patient records, ensuring adherence to data protection and confidentiality standards.
* Participate in practice meetings, clinical audits, and quality improvement initiatives to enhance patient care delivery.
*Requirements:*
* Registered Nurse with active NMC registration.
* Minimum of 2 years’ experience as a Practice Nurse in a primary care setting.
* Proficiency in chronic disease management, including diabetes, asthma, and hypertension.
* Strong communication, interpersonal, and teamwork skills.
* Ability to work autonomously and manage a diverse caseload.
