*Key Responsibilities:*
* Perform a range of clinical tasks, including health assessments, immunisations, wound care, cervical screening, and chronic disease management.
* Provide patient education and lifestyle advice to support health promotion and disease prevention.
* Develop and implement personalised care plans in collaboration with patients and other healthcare professionals.
* Support the practice team in maintaining high standards of care and ensuring compliance with clinical guidelines.
* Accurately document patient interactions and clinical interventions using electronic health records.
* Participate in quality improvement projects and contribute to the continuous development of practice services.
*Requirements:*
* NMC-registered Nurse with a minimum of 2 years’ experience in a general practice setting.
* Experience in chronic disease management and preventive care.
* Strong organisational skills and attention to detail.
* Excellent communication and patient engagement skills.
* Ability to work collaboratively within a multidisciplinary team.
