Columbia and surrounding areas.

In this role you will:
Ensure clients receive optimized levels of care to ensure favorable customer satisfaction
Communicate and coordinate care plans, adjustments, and special requests to all stakeholders
Scheduling of team to ensure continuity of care and adherence to plan
Motivation of team to achieve stated organizational goals and outcomes
