Role summary:
To provide compassionate and professional palliative nursing care to patients and their families as a part of the inter-disciplinary care team. Inform stakeholders of palliative care arrangements within the community-based service platform. Establish effective care pathways and linkage throughout the care continuum. Ensure effective management of palliative care clients in home and community-based settings.

Essential:
- CURRENT REGISTRATION WITH SANC AND COMPLETED COMMUNITY SERVICE
- PALLIATIVE CARE TRAINING
- VALID DRIVERS LICENCE
- MUST HAVE OWN VEHICLE
- Good communication skills, written and verbal.
- Strong organization skills, ability to multi-task.
- Well-developed computer skills (word, excel).
- Strong interpersonal skills.
