Daytime healthcare are working with an established Home Care agency, who are expanding and looking for an experience Care Coordinator to join their friendly team!

Duties include;
- Responsible for carrying out initial assessments, doing reviews and spot checks
- Creating service user care plans. Planning staff Rota s using the bespoke system
- Supporting the new and existing care staff team and coordinating the key carers in your designated areas
- Ensuring that all operations are Company and CQC Compliant
Requirements;
- You must have a full valid Driving Licence with access to a vehicle
- Good organisational and communication skillls to ensure an excellent service
- Experience doing rotas and care plans
